We support healthcare services for NHS hospital units, General Practices, Ministry of Defence (MoD) units and clinics within the Prison Services.

- Professional Registration No. GMC / GDC / NMC / HCPC
- NHS Performer Number (if applicable to the post)
- Photo ID with proof of address
- National Insurance number
- Passport
- Work Permit / RTW /Visa (if applicable)
- Proof of Indemnity Insurance
- Enhanced DBS
- Safeguarding Adults & Children Certificate Level 3
- Certificate of Completion of Training (CCT / JCPTGP)
- Evidence of CPD
- Other mandatory training certificates (BLS / ALS / Health & Safety)
- Full Health Screen / Immunisation history
